RESOLUTION NO R-93- 1 9
A RESOLUTION authorizing and directing the City Manager of the City of
Yakima to sign an Addendum to the 1992-1994 Collective
Bargaining Agreement with the Washington State
Council of County and City Employees Local 1122 of the
American Federation of State, County and Municipal
Employees, AFL-CIO as to amendments to the Division 7
(AFSCME) Healthcare Plan.
WHEREAS, pursuant to state law requirement, labor negotiations have
occurred between the City and the Washington State Council of County and
City Employees Local 1122 (AFSCME), and these negotiations have resulted in
an agreement on amendments to the Division 7 (AFSCME) Healthcare Plan,
which by the terms of the current collective bargaining agreement with that
union must be memorialized in an Addendum to the 1992-1994 Collective
Bargaining Agreement; and
WHEREAS, the City Council deems it in the best interest of the City
that the Addendum be completed at this time, now, therefore,
BE IT RESOLVED BY THE CITY COUNCIL OF THE C1TY OF YAKIMA.
THE City Manager is hereby authorized and directed to execute an
Addendum, to be attached to the 1992-1994 Collective Bargaining Agreement
by and between the City of Yakima, Washington and Washington State
Council of County and City Employees Local 1122 of the American Federation
of State, County and Municipal Employees, AFL-CIO, a copy of which
Addendum is attached and by this reference made a part hereof

ADDENDUM TO 1992-1994 COLLECTIVE BARGAINING AGREEMENT FOR
AFSCME EMPLOYEES
THIS ADDENDUM to the existing collective bargaining agreement is entered
into by and between the City of Yakima, Washington hereinafter called the
City, and Washington State Council of County and City Employees Local 1122
of the American Federation of State, County and Municipal Employees, AFL-
CIO, hereinafter called the Union.
The current 1992-1994 AFSCME contract provides in Article XVIII - Salaries
and Fringe Benefits, Section 18.B.4, that the only matter open for negotiations
for 1993 is the issue of insurance premium status by providing the following.
"18.B.4 The City of Yakima and AFSCME Local 1122 bargaining teams
will each select two (2) committee members each September to re-
examine the medical insurance premium status and make any
recommendations that seem warranted to both bargaining teams. The
bargaining teams must agree unanimously if any recommendations
are to be made to the City Manager."
The parties have met and negotiated a settlement of the single contract
reopener for 1993 insurance recommendations.
The parties have agreed that effective April 1, 1993, the following changes to
the health care plan for eligible Division 7 participants be implemented:
1. Prescription Drugs: Implement a prescription drug card
program.
2. Vision Examinations: Increase the eye examination allowance
from $45.00 to $60.00
3. Basic Benefits: Delete the $26.00 per day hospital room and board
charges which is paid at 100% and consider all hospital room and
board charges under the 80% major medical program.
4. Lifetime and Annual Maximums: Increase the lifetime
maximum from $500,000.00 to $750,000.00 and delete the annual
maximum of $250,000.00. Annual restoration remains at
$12,500.00.
5. Outpatient Mental Health Eliminate the maximum $20.00 per
outpatient visit, and retain the current $500.00 outpatient annual
maximum limit.
6. Pre -Existing Conditions: Change the definition of a pre-existing
condition from "symptoms which existed within 90 days before
coverage under this Plan began and which would cause an
ordinarily prudent person to seek treatment; or any symptoms
for which medical advice was recommended or treatment was
received within 90 days before coverage began under this Plan."

eliminate Section (1) " The last day of a 90 -day period during
which no treatment was received,"
7. Cosmetic, Plastic and Reconstructive Procedures: Add the
following language to the current benefit plan, "a child's
congenital anomoly must be corrected within 6 years of birth
and, unless born while covered under this plan, after a two year
waiting period." The provision would apply to eligible
claimants beginning benefits under this provision on or after the
effective date of this change.
Further, the parties have agreed that the Joint Committee on Health
Insurance continue to study the possibility of the Division 7 dental program
being included in the City's self insured dental program. If such a change is
proposed, it shall be changed at the earliest possible date.
Further, the parties have agreed that the Section 125 program, "Option" be
made available to AFSCME employees.

STATEMENT OF INTENT
The below -signed parties have the following understanding and
agreement on the meaning of the following sentence on page 2 of the attached
Addendum: "If such a change is proposed, it shall be changed at the earliest
possible date." The parties agree that this phrase refers to a preference that, if
a changeover is proposed by the Joint Committee, that the changeover be
effected as soon as possible. The parties agree that this phrase is not meant to
provide the Joint Committee decision-making authority that would eliminate
City Council approval of the actual provisions in the City's dental program
subsequent to AFSCME employees being included in the City's dental program.
This memorandum of understanding Is a part of the Addendum executed
by the parties on the day of /11,4 ," , 1993.
