WHEREAS, St. Elizabeth Medical Center maintains an Occu- pational Health Program for the purpose of making available certain preventive health care services to persons within the State of Washington; and WHEREAS, Yakima County contracted with St. Elizabeth Medical Center for said preventive health care services after completion of the competitive bid process; and WHEREAS, the City of Yakima desires to contract with St. Elizabeth Medical Center to provide certain preventive health care services to City employees; and WHEREAS, the City of Yakima has the authority under the Interlocal Cooperation Act to purchase services under the Yakima County bid process; and WHEREAS, the City Council deems it to be in the best interest of the City of Yakima to enter into the attached Occupational Health Services Agreement, now, therefore, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF YAKIMA: The City Manager is hereby authorized and directed to execute the attached and incorporated contract entitled "St. Elizabeth Medical Center Occupational Health Services Agree- ment" with St. Elizabeth Medical Center. ELIZABETH MEDICAL CENTER OCCUPATIONAL HEALTH SERVICES AGREEMENT AGREEMENT made as of June 1, 1991, by and between ST. ELIZABETH MEDICAL CENTER of Yakima, Washington, hereinafter referred to as "Medical Center" and CITY OF YAKIMA, hereinafter referred to as "Contractor". RECITALS WHEREAS, Medical Center maintains an occupational health program for the purpose of making available certain health services to persons within Washington, and WHEREAS, Contractor desires to contract with Medical Center to provide preventative health care services to its employees at locations as the parties shall agree; and WHEREAS, the parties enter into this Agreement to set forth their understanding in writing and to identify the types of services to be performed by Medical Center, the persons eligible for said services, and the compensation to be paid for the services. AGREEMENT NOW, THEREFORE, in consideration of the mutual covenants herein contained, the parties hereto agree as follows: 1. This Agreement shall commence June 1, 1991, and shall terminate on December 31, 1991. At least one month prior to the termination date, the parties shall meet and review the effectiveness of the services provided pursuant to this Agreement and to determine if this Agreement should be extended. If the parties agree to extend this Agreement, a new agreement shall be executed which shall set forth in writing the new term and all other matters incident to the agreement. 2. This contract may be terminated by either party by giving thirty (30) days written notice of such intent and will become effective thirty (30) days from the date such written notice is delivered to the applicable party to the contract. 3. Contractor agrees to pay Medical Center compensation for Medical Center's services rendered pursuant to this agreement. Compensation will be paid by Contractor at the following rates: A. Orientation Session $40.00 each session B. Individual Health Path Screening $40.00 each participant C. Result Workshop $40.00 each workshop D. Management Summary Report $50.00 E. Mileage @ 25.5 cents for actual miles traveled to complete health screening/follow-up visits for sites further than 10 miles from St. Elizabeth Medical Center, F. Professional Staff travel time - $30,00 per hour for actual time traveled to complete health screening/follow-up visits for sites further than 10 miles from hospital. G. Individual Goal Setting - R.N, @ $30.00 per hour, as arranged. H. Nurse follow-up visit - R.N. @ $30.00 per hour, as arranged. I. Health Education Classes -(Optional) $100.00 per hour, maximum fee. (plus travel time and mileage for sites further than 10 miles from hospital) Compensation is payable the month following receipt of billing. 4. Services to be rendered by Medical Center to eligible employees of Contractor are as follows: A. Orientation Presentations to employees regarding the who, what, when, where and why of individual Health Path Screenings and follow-up. B. Individual Health Path Screening Employees will be screened individually at the worksite or at St. Elizabeth Medical Center as the employee chooses. Screening will be provided at the worksites with the highest concentration of employees. There must be a minimum of 15 participants per site. Screening will include: 1. StayWell Health Risk Appraisal Questionnaire 2. Laboratory Blood Tests. Will include total cholesterol, HDL, and blood glucose. 3. Percentage of body fat using near -infrared light analysis. 4. Height, weight, and frame size. 5. Blood pressure and pulse 6. Physical assessment - (Heart and lung sounds, and skin inspection) by appointment at Occupational Health Department, St. Elizabeth Medical Center. 7. Fitness assessment - sit & reach, back extension, push- ups, and sit-ups 8. Educational Brochures 9. Health Path result book C. Group Result Workshops Each participating employee will receive a confidential written interpretation of their test results. An explanation of test results will be given to groups of employees as the contractor schedules. Test results will be mailed to employees' homes if they are unable to attend group sessions. D. Individual Goal Settina A health professional will meet individually with all interested employees to make recommendations for health improvements and assist each employee in setting specific health improvement goals. Each goal setting session will take approximately 20 minutes and will be provided at the worksite. Occupational Health will track employees' on- going health improvement efforts and will continue to offer advice, resources, referrals and encouragement to participating employees. E. Nurse Follow-up As scheduled by contractor, a nurse will visit screening sites for individual follow-up counseling and repeat testing. Repeat testing to be provided at no charge to the Contractor or the participant includes: 1. Height, weight, and frame size 2. Percentage of body fat 3. Blood pressure and pulse Participants will be charged $15.00 if they choose to repeat blood work. Blood test will include total cholesterol, HDL, and blood glucose and will be drawn at St. Elizabeth Medical Center lab. F. Educational Presentations (Optional) Topics and schedule to be determined by Contractor. G. Management Summary The Contractor will receive one set of the StayWell Management and Executive Reports after the goal setting sessions. This report will provide the following information: 1. Distribution of risk factors from HDA questionnaire. 2. Frequency of abnormal finding by type of test. 3. Type and disposition of referrals. 4. Categorization and frequency of all initial wellness goals. 5. Follow-up recommendations on specific wellness topics which should be offered to employees based on testing results. H. Additional Services/and fees As agreed between parties. 5. Medical Center is an independent contractor and in said capacity shall assume full responsibility for the management and supervision of its personnel while these individuals are rendering services pursuant to this Agreement. Contractor shall designate to the Medical Center the name or names of its responsible personnel to whom Medical Center personnel shall report in their performance of this Agreement. 6. This Agreement shall be construed and enforced in accordance with and governed by the laws of the State of Washington and shall not be amended, altered or changed except by written agreement signed by the parties hereto. 7. Political subdivisions of the State of Washington are allowed to purchase under Yakima County bids in accordance with RCW 39.34 Interlocal Cooperative Act. 8.
