SUMMARY EXPLANATION:
Tapani Inc completed the North 1st Street Revitalization N Street to SR12 Phase 1 Project
2340. Final inspection for this project was made and the recommendation is that the project be
accepted. A final payment of$0 is due from the City to Tapani Inc for the completion of this project.
Delayed closure of project due to Department of Labor& Industries review and staff turnover.

Contract Awarded on: 02/20/19
Contract Cost: $4,265,791.08
Total Due: $0
The total contract cost is for construction only and does not include engineering and other costs. City
Council action is required to accept the project and approve the final construction costs.

Physical Completion
Dear Mr. Hailing:
In accordance with Section 1-08.5 of the standard specifications this project is considered physically
complete as of Wednesday, December 16, 2019
The contract required the work to be physically complete within One Hundred and sixty (160) working
days. Five (5) additional working days were added by change order, for a total of one hundred sixty-five
(165) working days. Substantial Completion was granted on working day one hundred sixty-five (165)
working days. After which working days ceased to be counted in accordance with Section 1-08.5.
Physical Completion was granted on December 12, 2019 once the most of remaining punch-list items
had been completed.
Within the next 30 days we intend to issue the Final Contract Voucher Certificate(FCVC)for this contract.
We will need the following documentation from your office in order to issue the FCVC (list or attach list
as needed):
■ Statement(s) Affidavits of Wages Paid
Washington State Department of Labor and Industries (L&I) must certify that all Contractors and
Subcontractors who performed work on a public works contract are current with payments of their
industrial insurance.
